Case Managers
Chinese Information and Service Center is seeking CASE MANAGERS (multiple positions available) in Seattle, WA to interview and assess the clients medical health condition and confer with other professionals to evaluate the mental and physical condition of client; determine clients eligibility for state-funded in-home care and/or other case management support programs; conduct comprehensive physical and psycho-social assessment to develop an initial and long-term plan of care which enables the client to maintain their independence in their home and or community; formulate and develop a plan of care (service plan) that maximizes the physical, social, and emotional capabilities/health for the client; determine clients eligibility for state-funded in-home care services or other case management support programs; serve as the care coordinator between health agencies, hospitals, nursing facilities, public service agencies, and insurance programs; provide referral services (as necessary) to medical and non-medical public services and other in-home care services; provide case management services in multi-disciplinary approach for care service delivery; provide on-going case management/monitoring and adjust the plan of care as needed to accommodate the changing needs and physical, social, and emotional condition of each client; and provide ongoing case monitoring for vulnerable elderly and disabled adults who are functionally, mentally or physically unable to care for themselves by assessing and evaluating crisis situations. Minimum Requirements: Bachelors in Social Work, Sociology, Psychology or related field of study AND 24 months experience as Social Worker, Counselor or related social service experience.
