Advanced Practice Nurse Specialist - Health Care for the Homeless
This Clinical Quality Lead Position is designed to provide clinical nursing leadership to the Health Care for the Homeless Program. Specifically, this position will manage the program’s clinical quality assurance efforts in service of the program’s goal of assuring that the King County community is meeting the health needs of its homeless population. The position will work to assure compliance with standards of care and advance PHSKC’s capacity to set, measure, and advance clinical quality goals related to the provision of care both by the program’s contractors and by clinical programs operated by Public Health-Seattle and King County (PHSKC) that serve people experiencing homelessness. This work will require close collaboration with the HCH program manager, the HCH Contracts Manager, HCH Data and Evaluation Manager, managers of programs and sites providing direct clinical care, both internal and external to PHSKC, and staff across PHSKC’s Community Health Services (CHS), Prevention, and Environmental Health Divisions. Another central responsibility of this position is planning, organizing, and providing training and technical assistance on a variety of homeless health care topics to health care, housing, shelter, and social services providers serving the homeless. The position also participates in planning, organizing, and presentation activities related to prevention practices. Examples of these practices include those related to communicable diseases that can break out within shelters and encampments and adapting emergency preparedness and response to the needs of people experiencing homelessness. This position also works on planning, policy and program development related to broad community goals for improving outcomes and reducing risk for vulnerable populations.

- Job Duties
- In collaboration with multiple staff and stakeholders, assure compliance with standards of care and advance PHSKC’s capacity to set, measure, and advance clinical quality goals related to the provision of care by program contractors and by PHSKC-operated clinical programs serving people experiencing homelessness (35%).
- Plan, organize, and, at times, directly provide training and technical assistance for to health care entities (internal and external to Public Health), housing, shelter, and social services providers serving people living homeless. Seek to increase the rate of adoption of evidence-based and promising practices in adapting models of care to barriers and challenges that are significantly more pronounced within the homeless population than in the general population. Provide leadership and give input into the creation and dissemination of best practice guidelines and protocols for homeless health topics.
Examples of adaptive practices include patient-centered, culturally and linguistically appropriate, and trauma-informed care, motivational interviewing, anti-racist/pro-equity practice, and harm reduction. The training topics will also be adaptable to low-barrier approaches and account for differences in shelter/housing-based care vs. outreach-based care along with the principles of care coordination for patients with complex co-morbidities, and secondary trauma for service providers. (20%).
- Provide leadership and give input into the creation and dissemination of best practice guidelines and protocols for homeless health topics.
- Participate in planning activities at the program and community level, e.g., participating in community work groups and collaborating with program managers and contracts administrators (15%)
- Develop and deliver presentations related to best and promising practices for adapting care for people living homeless (15%)
- Support the work of the HCH Program staff and the program’s Community Advisory Group in responding to current and emerging issues related to reducing health-related risk for homeless and other vulnerable populations (10%)
- Supporting grant proposal development (5%)
- Advanced knowledge of health care delivery shown to be effective in improving access to care and health outcomes for people currently and formerly experiencing homelessness;
- Advanced knowledge of public health nursing process and principles, including needs assessment process at the systems and community level;
- Knowledge of service systems serving homeless people including health care programs, shelter, day centers, various housing programs including permanent supportive housing.
- Demonstrated skill and experience in quality improvement activities in a health care setting;
- Demonstrated skill and experience in community organization and coalition building strategies and techniques;
- Advanced knowledge of health promotion and disease prevention principles as well as understanding data and its use in training and TA activities;
- Demonstrated skill and experience in using electronic health records, including understanding of extracted data;
- Demonstrated skill and experience in developing and delivering training presentations to audiences at all levels of familiarity with disease prevention, service delivery, population characteristics and models of care;
- Knowledge of: Washington State law relating to nursing care (WAC 18.79) and federal health care regulations; professional ethics related to delivery of nursing and allied health services;
- Demonstrated experience and skill in multicultural and interdisciplinary coalition building;
- Advanced knowledge of health promotion, risk assessment and education techniques and principles; health status indicators and variability within populations; community, professional and educational resources; adult learning principles; project management techniques; leadership practices;
- Demonstrated ability to communicate and maintain working relationships with diverse populations; facilitate discussion and consensus; make public presentations.
- Knowledge of equity and social justice principles and practices and their application in work environments.
- Washington State Registered Nurse License in good standing.
- Bachelor of Science in Nursing (BSN) from an accredited school of nursing or equivalent.
- Proof of Current BLS CPR Certification for Healthcare Providers (Please attach if current.)
- Washington State Driver's License or ability to travel within King County.
- Bilingual and/or bicultural
- Master’s degree in nursing or public health and/or BSN with extensive training and experience in the specialty area of homeless health nursing

Work Schedule: The individual selected for this position will have a flexible work schedule typically between the hours 8:00 a.m. to 5:00 p.m. Monday through Friday. This position is overtime eligible and may work outside of the typical schedule to meet operational needs.
Work Location: The home base for this position is the Chinook building, 401 5th Ave., Seattle, WA.
The work associated with this position may be performed through teleworking with reporting to an office location and to meetings as needed. Employees will have access to shared workspaces at various King County facilities. Employees must reside in Washington state and within a reasonable distance to their King County worksite to respond to workplace reporting requirements.
Employees will be provided with a County issued laptop and must maintain a workspace with an internet connection (access may be supplemented in some situations) where they can reliably perform work and remain available and responsive during scheduled work hours. Please note that when an employee conducts work that is likely to bring them in contact with another individual, safety precautions are required, including the wearing of masks in some situations.
