Summary



The Seattle King County Mobile Medical Program is seeking a skilled, committed, and trauma-informed Advanced Registered Nurse Practitioner (ARNP) to be part of the Seattle Street Medicine Team. This position will have an explicit focus on supporting clinical efforts at the REACH Markham building as the result of a new collaboration between REACH and PHSKC to enhance clinical capacity on site at the Markham. This position is 0.8 FTE, but a lower FTE may be negotiated as needed, with a minimum of 0.6 FTE. The ARNP will join the multidisciplinary team to collaboratively provide health care to individuals experiencing homelessness in King County. In this position, the ARNP will work with public health nurses, harm reduction/behavioral health specialists and community health outreach workers to provide medical care at the Markham building and in the field. The Mobile Medical Program provides non-emergent acute and chronic care, family planning, STI evaluation and treatment, low-barrier medication treatment for opioid use disorder, harm reduction services, and linkage to community-based primary and mental health care. This position may also provide coverage or capacity to other Mobile Medical outreach based teams. Street-based care is provided at various outdoor locations throughout the county. In general, a Sprinter van is used for transportation to and from street-based sites and care is provided both inside and outside of the Sprinter van.

Job Duties

  • Provide care as an Advanced Registered Nurse Practitioner in collaboration with a multidisciplinary team utilizing a patient-centered and harm reduction approach.
  • Provide clinical triage and patient care evaluation. Perform appropriate physical examination and assessment.
  • Order or perform appropriate diagnostic and screening tests and collect additional data for evaluation of illness/wellness. Develop differential diagnosis and risk identification through assessment and interpretation of data.
  • Educate and counsel patients on a broad scope of health care issues.
  • Provide clinical technical consultation to nursing staff, other clinical team members, and the community including advocacy for patients.
  • Document all medical evaluation, diagnostic procedures, treatment, outcomes, referrals, and consultations in a timely manner.
  • Perform minor procedures, point of care testing, and other duties as assigned within the position's scope.
  • Participate in ongoing program and clinical improvement, in collaboration with Mobile Medical team members and other Public Health staff.
  • Other duties as assigned.

Experience, Qualifications, Knowledge, Skills



Requirements:
  • Washington State license as a Registered Nurse in good standing.
  • Washington State license as an Advanced Registered Nurse Practitioner in good standing with prescriptive authority.
  • A bachelor's degree in nursing and/or a graduate level nursing degree of MSN, DNP or PhD from an accredited school of nursing.
  • Completion of advanced practitioner training and board certification as a nurse practitioner within a relevant specialty practice.
  • National Provider Identification (NPI) number required prior to credentialing.
  • Drug Enforcement Administration (DEA) registration required prior to credentialing.
  • Documentation of current BLS CPR certification with AED certification for Healthcare Providers is required prior to credentialing.
  • Valid Washington State driver's license or have the ability to travel throughout King County in a timely manner.
  • Willing to provide options counseling that could include referral for termination.
  • Pass background check, pre-employment physical and licensed independent practitioner credentialing

Work Schedule: The work schedule for this 0.8 FTE position is four, eight-hour shifts per week (ie: Monday through Thursday or Tuesday through Friday). In general hours are between 8am and 5pm. However, the position may work outside typical days/hours/sites to meet operational needs. This is a fully benefited position and is overtime eligible.

Interviews for this opportunity will be February 20th and 21st and will be used to fill all needs.

Working Conditions: Work is conducted in a variety of locations including indoors in office, on a vehicle, and outdoors. Travel throughout King County to various locations where unsheltered people are living is required. Outreach work is conducted in uncontrolled outdoor environments and will require the employee to traverse over uneven surfaces and obstacles, ascend and descend slopes, and move through wooded areas without maintained trails while transporting gear weighing up to 35 lbs. The employee will frequently work in and around extreme heat and cold, precipitation, ambient smoke, loud noises, individuals who are distressed, intoxicated, or displaying unpleasant behavior, and potentially biohazardous material. Must constantly perceive surroundings for and communicate with team about potential hazards under these conditions. Appropriate training and PPE will be provided. Must be able to communicate clearly and quickly with team members and clients in English and be responsive to phone calls and messages throughout the day. Must remain calm in adverse conditions and respond appropriately to emergency situations as they arise following established safety protocols. Employee may be called upon to administer naloxone and / or provide basic life support care to individuals in emergency situations, alone or as a member of a team. Employee will be required to share responsibility for driving the outreach vehicle(s) up to 60 miles per day in all weather conditions. The employee will also be required to move boxes, supplies, and packs weighing up to 35 lbs. between ground level and 5ft to load and unload outreach supplies.

Work Location: This position will be primarily based out of the REACH Markham building, located at 2133 3rd Ave Suite 116, Seattle, WA 98121. However, this position may also provide coverage to field-based teams serving various sites throughout the county. The Mobile Medical Program Office is located in South Seattle at 7272 W Marginal Way S Seattle, WA 98108.
