Term-Limited Temporary positions are "at-will" positions but are eligible for benefits. This TLT is currently anticipated to last between six (6) months to one (1) year. Extensions may be likely to accommodate evolving circumstances in the response to COVID-19. However, funding sources for this work remain uncertain and dependent upon decisions actively being made at both the federal and local levels. As a result, the end date of this position may be adjusted based on those decisions. The County and the Department continue to work to secure long term funding sources.This vacancy can also be filled as a Special Duty opportunity.
For Special Duty Assignment interest, you must be a Career Service, Career Service Exempt or other regular (non-temporary) employees in King County who have completed their initial County probation. You must have supervisor approval prior to taking a Special Duty Assignment.

The CHW (Community Health Worker) Care Coordinator Program Manager II (PPM II) position will support the operations of the CHW Care Coordinator services in Public Health’s COVID-19 response, with ultimate responsibility for directing and supervising the work of the CHWs.
The CHW Care Coordinator Program promotes the health and well-being of people and families who are required to isolate or quarantine due to COVID-19 diagnosis. The program is centering BIPOC (Black Indigenous People of Color) who are most impacted by the intersecting crises of racism and the pandemic. The primary goal is to provide support and resources to assure individuals and families can stay in their homes and heal with dignity. We are responding to the impacts of isolation and quarantine by providing:
- COVID-19 specific isolation and quarantine support
- Routine social and health care support
- Ongoing emotional and physical health care resources

The work associated with this position will be performed remotely, at least through July 5, 2021 in compliance with King County’s telecommuting mandate. It is possible that the work will be primarily performed remotely on an ongoing basis after July 5, 2021. Employees will be provided with a County issued laptop and must maintain a workspace with an internet connection where they can reliably perform work and remain available and responsive during scheduled work hours. Employees must reside in WA state and within a reasonable distance to their King County worksite to respond to workplace reporting requirements. There will be situations where the employee is required to report to a County worksite. Please note that when an employee conducts work that is likely to bring them in contact with another individual, safety precautions are required by the department in alignment with the Public Health Directive from the Seattle & King County Public Health Officer.

Work Schedule:
Weekend and evening work is required, and some local travel may be required. Workdays and hours will be determined at time of hire, may vary based on business needs, and may include some evening and weekend hours. Regular remote work may be required, tools necessary to successfully work remotely will be provided. Home site is King County Chinook Building in Seattle, WA.
- Job Duties
- Provide daily support and clinical direction in personnel management and operations of the innovative CHW Care Coordinator Program to support the physical and emotional health of individuals and families living in isolation or quarantine due to COVID19.
- Support care coordination in collaboration with the contact tracing team with an approach that best meets the needs of communities affected by COVID-19, with a primary focus on communities of color, immigrant and refugee communities, and those with limited English proficiency.
- Serve as the technical expert for care coordination for families in isolation or quarantine for COVID-19.
- Cultivate and sustain relationships with community partners supporting the health of our community at large and those living with the impact of COVID19.
- Ensure culturally responsive and relevant information and resource sharing related to recovery from COVID19, general health and community resources.
- Contribute to development and monitoring of program budgets, contractual agreements and resources. Assist in justification of project, program and/or agency budget requests.
- Organize and co-facilitate team meetings.
- Organize and collaborate with program managers to design and present staff trainings and identify additional training needs.
- Liaise with KC Prevention Section and COVID19 Community Mitigation & Recovery efforts, as well as other county programs related to community physical and emotional health.
- Represent Public Health with internal and external partners, including community stakeholders.
- Promote equity by fostering an equitable, diverse and inclusive work environment.
- Model and promote a positive, supportive culture through effective communication, leadership, guidance and support.

- Experience, Qualifications, Knowledge, Skills
- Knowledge of equity and social justice principles and practices, and understanding of the effects of place, race and policy and systems-based inequities on communities and populations.
- Interest in and/or professional or personal experience in supporting health recovery and health promotion in communities most impacted by the public health crises of racism & COVID19.
- A combination of at least 3 years of education and clinical experience within health, public health, community empowerment, and/or other related fields with emphasis on promoting health in communities and supervising staff.
- Experience supervising multi-disciplinary staff.
- Demonstrated history of successfully delivering projects on time and within budget.
- Demonstrated excellent writing and communication skills.
- Skills and abilities working collaboratively with diverse stakeholders including families, culturally specific and community-based groups, service providers, and co-workers.
- Ability to work with creativity and flexibility, both independently and as a team member.
- Proficient MS Office skills, such as Excel, Word, Outlook, and PowerPoint.

- BA/BS degree in social services, social work, other related field.
- Demonstrated experience working with underserviced communities in King County.
- Awareness and knowledge of resources in King County related to social and health resources.
- Understanding of intersectionality and how it impacts BIPOC (Black Indigenous People of Color) communities and their health.
- Demonstrated problem-solving and trouble-shooting experience.
- Skilled in relating to and working with clients from various cultures, racial and social and economic status.
- Experience managing or directing the work of a diverse workforce.
