Summary
Public Health's Prevention Division is looking for a Disease Research Intervention Specialist (DRIS) to join the Hepatitis C (HCV) Program. The HCV DRIS, under the supervision of the HCV program manager, will provide HCV education, referrals and care coordination for patients living with HCV. This work will include 1) identifying patients with HCV infection in need of counseling, additional testing and/or referral to HCV-related medical care, 2) investigating cases of HCV to determine if they still reside locally and their care status, 3) conducting outreach, assessing patient needs, and providing education and care coordination for HCV-related medical care, 4) collaborating with healthcare professionals and community-based organizations to coordinate medical care and additional support services, and 5) documenting outreach and engagement activities.
The ideal candidate should be good at multitasking, creative in solving problems, tenacious/persistent, good at adjusting communication styles as needed, and skilled in having difficult conversations. Ideally, the chosen candidate will have experience working with people experiencing homelessness, mental health and/or substance use disorders, a strong understanding of harm reduction principles and trauma-informed care, and knowledge of HCV care and prevention. Knowledge of, or ability to rapidly learn electronic health records and REDCap is also desirable.

WORK LOCATION: The main project office is at Chinook Building located at 401 5th Ave, Seattle WA 98104. The work associated with this position will be hybrid.
Employees will have access to shared workspaces at various King County facilities. Employees must reside in Washington state and within a reasonable distance to their King County worksite to respond to workplace reporting requirements.
Employees will be provided with a County issued laptop and must maintain a workspace with an internet connection (access may be supplemented in some situations) where they can reliably perform work and remain available and responsive during scheduled work hours. Job Duties
- Working with the HCV epidemiologist, use surveillance data to conduct HCV investigations designed to identify people with HCV who require treatment
- Review laboratory and electronic health record (EHR) reports to identify patients with HCV infection in need of counseling, additional testing and/or referral to HCV-related medical care
- Conduct outreach to patients via phone, text, and/or email to provide patient education on HCV, including harm reduction, disease management, medication management, medication adherence and healthcare-related issues
- Conduct patient needs assessments to determine barriers to accessing HCV care and facilitate referrals for additional support services
- Practice cultural humility and trauma-informed care when conducting patient interviews
- Seek out interpreter services and collaborate with community partners as needed
- Communicate with medical providers, nurses, social workers, and other service providers to coordinate referrals to medical care and support services
- Collaborate with Public Health - Seattle & King County (PHSKC) programs serving similar patient populations (i.e., Healthcare for the Homeless, HIV/STD Clinic, and Downtown Needle Exchange) to coordinate care of mutual patients and facilitate referrals to community-based services
- Document investigations and outreach encounters in multiple databases, following strict data security and confidentiality protocols and ensuring completeness and quality of data
- Attend weekly case supervision meetings with HCV program manager to discuss patient care plans and strategies for addressing barriers to care
- Under the supervision of the HCV program manager, provide consultation, outreach, education, training, and public messaging about HCV to healthcare professionals and community partners
- Participate in equity and social justice (ESJ) trainings and racial caucusing to support King County's commitment to antiracism

Required Experience, Qualifications, Knowledge, Skills
- Experience with harm reduction principles and trauma-informed care
- Demonstrated interviewing and elicitation (the act of obtaining, drawing out, or bringing out responses of others) skills.
- Experience with interviewing and collecting sensitive information (i.e., substance use, sexual practices, etc.) from patients
- Effective communication, interpersonal, organizational, and time management skills
- Motivation and self-direction; ability to prioritize competing responsibilities and meet projects goals and deadlines
- Understanding of individual, community and structural factors that influence disease prevention and engagement with healthcare services
- Sensitivity to populations at increased risk for HCV transmission (people who use drugs, people engaged in commercial sex work, people with a history of incarceration, and people experiencing homelessness)
- Ability to establish trust with people with mental illnesses, substance use disorders, housing instability, of varying sexual orientation & gender identities, racial/ethnic backgrounds, and socioeconomic status
- Computer competency, including the ability to enter and access data relevant to program services
- Ability to comply with strict protocols for data security and confidentiality
- Strong attention to details and accuracy
- Documentation of current CPR/BLS certification with AED training required at time of hire. CPR/AED Heartsaver courses do not meet this requirement
- Experience with and knowledge of HCV and related medical care and prevention
- Experience conducting public health outreach and investigations
- Knowledge of King County support services and experience coordinating supportive services
