Disease Research and Intervention Specialist - Pilot Mobile Outreach Team
- Summary
This position will be based at the Sexual Health Clinic at the Harborview Campus, though the work will primarily be in the field. This position will work as part of a mobile team to provide outreach, referrals, medication delivery and more to people living with HIV/AIDS (PLWHA) who have never engaged in HIV medical care or have a history of intermittent HV medical care with poor adherence to medication. This position tries to reduce barriers to engagement in HIV care for vulnerable PLWHA, who are often living homeless, have mental illness and substance use disorder. This position will base their outreach in downtown Seattle and south King County.
This recruitment may be used to hire STT, TLT, Career Service for the next 6 months.

- Outreach appropriate PLWHA through field visits, including visits to supportive housing facilities, other residences, and homeless encampments along with other mobile team members
- Outreach appropriate PLWHA through telephone calls, text messaging, visits to the hospital and other sites of care. Provide services as needed to support engagement in HIV care and behavioral health care, including adherence support, general psychosocial support, care coordination and direct services in the field.
- Establish and build relationships with organizations that serve PLWHA who are not engaged in medical care or virally suppressed. (supportive housing, shelters, jails, hospitals, orgs who provide mental health and substance use monitoring/treatment)
- Work with Mobile team to prioritize, consult and fine-tune outreach efforts. This is a pilot project and will be constantly fine-tuning/pivoting to ensure best outcomes
- Provide venipuncture to get HIV and other labs.
- Provide information, education and support to PLWHA to remove barriers to engagement in care/medications. (This could include delivering medications, transporting/escorting people to medical or other visits, providing incentives, etc.)
- Work with MAX Clinic and other agencies to take referrals for outreach
- Provide psychosocial support tailored to patients’ level of need
- Provide information on other infections: Hepatitis A, B, C, STIS, etc.
- Educate community providers and agencies about Mobile team outreach
- Complete data documentation as needed
- Investigate cases to collect information
- Complete paperwork as needed to track incentives and give to Program Coordinator
- Process patient specimens and transport the specimens to the HMC lab for testing
- Participate in frequent team huddles to communicate critical patient updates that require follow-up

REQUIRED EXPERIENCE, QUALIFICATIONS, KNOWLEDGE, SKILLS:
- Ability to approach potential clients in a variety of community settings and quickly establish rapport in-order-to work with them to consider engagement in medical care/HIV meds
- Experience working in a fast paced, rapidly changing environment while juggling competing priorities and maintaining composure and safety.
- Ability to collaborate effectively with a certified Peer (someone with lived experience with one of: HIV, homelessness, substance use, mental illness) as an outreach partner.
- Demonstrated experience working with people experiencing substance use, homelessness and mental health concerns and overall behavioral health issues from a harm reduction framework
- Demonstrated interviewing and elicitation (the act of obtaining, drawing out, or bringing out responses of others) skills.
- Knowledge and demonstrated application of motivational interviewing and motivational skills.
- Ability to protect Personal Health Information (PHI) and other confidential information.
- Ability to follow procedures/protocols and apply them to unpredictable situations.
- Demonstrated ability to work effectively with people of diverse backgrounds.
- Effective and respectful verbal and written communication skills.
- Knowledge, skills and abilities in the areas of medical terminology, diagnosis and treatment.
- Ability to establish and maintain effective, respectful working relationships.
- Able to speak and write effectively resulting in good notes and reports.
- Ability to work independently and exercise good judgment in making decisions.
- Experience with computer applications including Word, Excel, Access and web browsing.
- Possess a valid WA State driver's license or ability to obtain one within 90 days of hire and willing and able to drive a county vehicle in the day and nighttime hours.
