Summary
This position will work out of the Sexual Health Clinic at the Harborview Campus . This position provides non-medical case management for clients of the Max Clinic at the Public Health -Seattle & King County Sexual Health Clinic. Max Clinic is an invitation only, drop in, incentivized HIV clinic. The goal is to reach the most vulnerable populations that either have never engaged in HIV medical care or have a history of intermittent HV medical care with poor adherence to medication. Max Clinic tries to reduce barriers of engagement in HIV care. The patients in Max Clinic are mainly living homeless, have mental illness and substance use disorder. This TLT position is anticipated to last through 12/31/2023.

Job Duties
- Room and assess a patient's needs when they arrive in clinic.
- Recruit referred patients into clinic through telephone calls, letters, field visits, and hospital visits.
- Educate new patients about the Max Clinic and ensure that appropriate enrollment paperwork is filled out.
- Outreach to patients due for follow-up visits.
- Escort Max patients to appointments, blood draws, immediate imaging, ED admissions, etc.
- Respond to text and jail alerts for Max Clinic patients or potential Max Clinic patients.
- Create and sustain relationships with emergency room social work teams at Northwest Hospital, UWMC and HMC to navigate systems to efficiently approach potential Max patients.
- Serve as a liaison for each Max Clinic patient and help them to navigate the broader health care system as well as medical care within the Max Clinic
- Provide psychosocial support tailored to patients' level of need.
- Trouble shooting and addressing barriers to care and adherence.
- Coordinate with other case managers and community providers to support engagement in care.
- Coordinate with Department of Corrections/King County Correctional Facility release planning team.
- Provide education and counseling re: HIV and medication adherence.
- Educate community providers and agencies about Max Clinic.
- Assess services needed that day for each patient and prepare Max Clinic patients to see the provider-scheduling in EMR, asking nursing team to obtain vitals.
- Manage payment and accounting for gift cards and bus passes for patients.
- Process patient specimens and transport the specimens to the HMC lab for testing.
- Maintain the safety of the clinic environment by implementing appropriate infection control protocols, responding to inappropriate patient behavior and deescalating patients in crisis.
- Pick-up, safely store and give prescribed medications to patients in clinic.
- Participate in weekly team huddles to communicate critical patient updates that require follow-up.
EXPERIENCE, QUALIFICATIONS, KNOWLEDGE, SKILLS:
- Ability to approach potential clients in a variety of community settings and quickly establish rapport in order to engage them in HIV/STD services
- Ability to work in a fast paced, rapidly changing environment while juggling competing priorities and maintaining composure and safety.
- Ability to collaborate effectively with an interdisciplinary care team of DRIS, social workers and medical providers
- Demonstrated experience working with people experiencing substance use, homelessness and mental health concerns and overall behavioral health issues from a harm reduction framework
- Demonstrated interviewing and elicitation (the act of obtaining, drawing out, or bringing out responses of others) skills.
- Ability to provide HIV health information simply and concisely.
- Knowledge and demonstrated application of motivational interviewing and motivational skills.
- Ability to follow procedures/protocols and apply them to unpredictable situations.
- Demonstrated ability to work effectively with people of diverse backgrounds.
- Effective and respectful verbal and written communication skills.
- Knowledge, skills and abilities in the areas of medical terminology, diagnosis and treatment.
- Ability to establish and maintain effective, respectful working relationships.
- Ability to work independently and exercise good judgment in making decisions.
- Experience with computer applications including Word, Excel, Access and web browsing.
- Possess a valid WA State driver's license or ability to obtain one within 90 days of hire and willing and able to drive a county vehicle in the day and night time hours.
- Fluency in a language other than English
- Demonstrated experience working with electronic medical records and conducting chart reviews to investigate cases or coordinate patient care
- Experience with data entry.
