Downtown Public Health Center is looking for a skilled Public Health Nurse (PHN) for a Career Service position.  We are looking for someone who brings experience in working with diverse, complex, vulnerable adults with histories that include trauma, behavioral health concerns, substance use disorder, and living homeless. This nurse will work in the Pathways program housed in the Downtown Public Health Center, in Seattle's Belltown? ?This program is a collaborative care model designed with a harm reduction approach to help address the opioid epidemic and provide low barrier access for individuals to medications to assist with opioid use disorder.  Our clinic is in the process of also expanding to provide essential primary care services to anyone with substance use disorders. This PHN provides primary care case management and will coordinate with multiple programs within the facility and community to provide clients with seamless care. This PHN will be instrumental in the development of individual's care plans. Responsibilities include screening, assessment, treatment monitoring and interventions, patient care planning, evaluation, and medical care coordination. This PHN will provide nursing education to clients, their family members, or other supportive individuals about Medication Assisted Treatment for Opioid Use Disorder as well as risks, benefits, potential side effects, interactions, program requirements, consents, and treatment contracts. This nurse will work collaboratively with clients and community to build a network of support for the client to reach their goals.  This nurse will involve clients in the development of their care plans, and work collaboratively with the clients to address their needs. The nurse maintains the clinic environment, participates in developing policies and procedures, coordinates the care team's efforts and maintains a positive relationship with the clients served.?The nurse will also provide outreach into the community, partnering with both Public Health and community outreach teams to support building a bridge between outreach and center-based services.

Job  Duties: 

  • Obtain and document health history and make nursing diagnosis based on nursing assessment.  

  • Documentation requires electronic charting: EPIC.  

  • Provide support services to the client, referring to outside resources when necessary.  

  • Participate in quality assurance activities to meet laws, regulations, policies and procedures.  

  • Provide training, orientation and education to staff, clients, students and the community. 

  • Develop and maintain on-going partnerships with community outreach and healthcare providers to support collaborative care. Will include some off-site coordination and collaboration with outreach programs. 

  • May provide birth control and pregnancy options counseling. 

  • CoVID response – duties will include testing and vaccinating clients for CoVID-19 

  • May be redeployed to CoVID response duties as needed. 



  • Knowledge of nursing principles and processes; human systems; infectious disease management and control; health care system structure and function.  

  • Knowledge of adult learning and health education techniques and principles.  

  • Knowledge of personal safety standards, safety techniques and principles.  

  • Demonstrated abilities performing comprehensive nursing assessments and successfully engaging clients in a plan of care.  

  • Demonstrated ability to communicate effectively with patients and their families and with other health and human services staff; developing and maintaining therapeutic relationships with patients; and working with culturally and socio-economically diverse populations.  

  • Demonstrated ability providing reproductive health education, services, community outreach, and working with community partners and agencies.  

  • Demonstrated ability working in a fast-paced environment independently, multi-tasking, demonstrating attention to detail, crisis intervention and problem solving skills and effective time management.  

  • Ability to work collaboratively with multidisciplinary teams to improve patient outcomes.  

  • Demonstrated ability completing medical record documentation, problem-solving and case management.  

  • Demonstrated organizational and leadership abilities.  

  • Willing to provide options counseling.  

  • Skilled in motivational interviewing  

  • Able to provide care through a lens of harm reduction, trauma informed care and equity & social justice. 




  • Washington State Drivers' License or the ability to provide transportation to work. 

  • Current BLS-CPR for Healthcare Providers 

  • Current Washington State Registered Nurse license 

  • Bachelor's Degree in Nursing from an accredited school of nursing.

  • Willing to provide options counseling 

  • Letter of interest describing your interest in working with this population


  • Bilingual and/or bicultural  (Spanish/English preferred)

  • Lived experience with homelessness, substance use disorder 

  • Knowledge of vaccine administration 

  • Familiar with EPIC System 

  • Demonstrated ability to work in community settings with culturally and socio-economically diverse populations

  • Demonstrated ability to apply concepts of trauma-informed care and equity & social justice principles to clinical practice 

  • Ability to build community coalitions

  • De-escalation skills

  • Flexibility and creativity

Typical work hours are Monday-Friday 8 a.m. to 5 p.m., with flexing into evenings and weekends to support outreach work as needed to meet operational needs. This position is fully benefitted and overtime eligible.
