Public Health Nurse - Kids Plus Program
- Summary
As a member of a multi-disciplinary team, this Public Health Nurse (PHN) on the Kids Plus team provides holistic services to low income families experiencing homelessness (street, shelter, doubled up, transitional housing) through outreach and case management. This PHN partners with families from diverse backgrounds to identify their strengths, needs, goals and support systems. Following the family's lead and honoring the family's cultural and linguistic diversity, the PHN coordinates care, offers health education and screening, provides referrals to resources, and offers support to decrease the stress of homelessness for the entire family. This PHN may also participate in community groups, coalitions and advisory committees to improve services available to families experiencing homelessness.

Work location & schedule: This position is based at Downtown Public Health Center located at 2124 4th Avenue, Seattle, WA 98121 and will work a 40-hour work week. The typical schedule is from 8:30 a.m. to 5:30 p.m. but could work outside of this schedule based on operational needs.

PHN must be comfortable working in these environments.
- Provide client centered care to people from diverse backgrounds who frequently have a history of trauma.
- Work as a member of a multidisciplinary team of social workers and PHNs.
- Provide services to families through both outreach and/or case management models.
- Work with each family to identify and address their priority concerns and develop strategies to address those concerns.
- Provide health services: care coordination, linkage and referral, chronic disease management, developmental screenings and referrals.
- Provide support regarding a wide array of issues at family's discretion (basic needs, schooling and childcare, substance use, mental health concerns, education and employment, connection to peer and social support, domestic violence advocacy, and more).
- Knowledge of public health nursing principles and processes; human systems; infectious disease management and control; health care system structure and function.
- Knowledge of child and adult learning and health education techniques and principles.
- Knowledge of personal safety standards, safety techniques and principles.
- Demonstrated abilities performing comprehensive nursing assessments and successfully engaging clients in a plan of care.
- Demonstrated ability to work in various community settings (including in outdoor encampments and shelters in all weather) without on-site supervision; communicating effectively with patients and their families and with other health and human services staff; developing and maintaining therapeutic relationships with patients; and working with culturally and socio-economically diverse populations.
- Demonstrated ability providing reproductive health education, services, community outreach, and working with community partners and agencies.
- Demonstrated ability working independently in a fast paced environment, multi-tasking, demonstrating attention to detail, crisis intervention and problem-solving skills as well as effective time management.
- Demonstrated abilities in areas of medical record documentation, problem-solving, and case management.
- Demonstrated organizational and leadership abilities.
- Willing to provide options counseling.
Desired qualifications:- Experience working with families with mental health, substance use or interpersonal violence concerns
- Ability to apply equity and social justice principles in the work place
- Experience working on a multidisciplinary team with evidence of effective collaboration skills
- Bilingual/Bicultural
Licensing/certification requirements:- Washington State Registered Nurse License in good standing
- Bachelors of Science in Nursing (BSN) from an accredited school of nursing or equivalent
- Current Washington State Driver's License
- Documentation of current BLS CPR certification with AED training required at time of hire. CPR/AED Heartsaver courses do not meet this requirement.

- Public Health relies on office automation (Microsoft Office) and web-based enabled tools, therefore candidates must be proficient and comfortable with computer use to perform functions associated with on-going work.
- Regular and reliable attendance, effective communication skills, and development of effective working relationships are requirements of all Public Health positions.
- Staff may be required to play an active role in the event of a public health emergency, which may include changes in responsibilities and working hours.

- Employees are required to adhere to OSHA/WISHA guidelines including but not limited to completing their mandatory trainings on time.
