Public Health Seattle-King County Health Care for the Homeless (HCH) Mobile Medical Program Street Medicine team is hiring 3 career service Public Health Nurses to join our Street Medicine Team. We are seeking Registered Nurses who are self-directed, creative, flexible and skilled in field based clinical work as well as being committed to an anti-racist and harm reduction nursing practice. Each position is 0.8 FTE, with potential for increased FTE based on business need. The Street Medicine Team seeks out and brings medical, behavioral health care and social services directly to people living outside, i.e.: "rough sleepers". The Street Medicine team aims to engage people in health education, professional and peer support, and treatment for their medical and behavioral health conditions via a trauma-informed, antiracist and harm reduction approach. In this role, the PHN will work with the multidisciplinary Street Medicine Team that addresses the medical, behavioral health care and harm reduction needs of people living homeless. The PHN level work includes nursing assessments, wound care, point of care testing, COVID testing, vaccinations, and provision of test results. This PHN may also be assigned to shifts at the REACH Markham in support of a new collaboration between REACH and PHSKC to enhance clinic onsite clinical capacity at the Markham building. This role may occasionally provide support to other current or future Mobile Medical Teams providing float coverage as well as to increase capacity for conducting nurse visits. The ideal candidate will have sound experience in providing nursing care in ambulatory care and outreach settings.

Working Conditions: Work is conducted in a variety of locations including indoors in office, on a vehicle, and outdoors. Travel throughout King County to various locations where unsheltered people are living is required. Outreach work is conducted in uncontrolled outdoor environments and will require the employee to traverse over uneven surfaces and obstacles, ascend and descend slopes, and move through wooded areas without maintained trails while transporting gear weighing up to 35 lbs. The employee will frequently work in and around extreme heat and cold, precipitation, ambient smoke, loud noises, individuals who are distressed, intoxicated, or displaying unpleasant behavior, and potentially biohazardous material. Must constantly perceive surroundings for and communicate with team about potential hazards under these conditions. Appropriate training and PPE will be provided. Must be able to communicate clearly and quickly with team members and clients in English and be responsive to phone calls and messages throughout the day. Must remain calm in adverse conditions and respond appropriately to emergency situations as they arise following established safety protocols. Employee may be called upon to administer naloxone and / or provide basic life support care to individuals in emergency situations, alone or as a member of a team. Employee will be required to share responsibility for driving the outreach vehicle(s) up to 60 miles per day in all weather conditions. The employee will also be required to move boxes, supplies, and packs weighing up to 35 lbs between ground level and 5ft to load and unload outreach supplies.
Work Location: The Mobile Medical Program Office is located in South Seattle at 7272 W Marginal Way S Seattle, WA 98108 but these positions will work at various sites throughout Seattle-King County.

Job Duties
- Provide outreach and direct nursing care to people living homeless in community settings including day centers, encampments, and outside in the field.
- Facilitate linkages to primary care, behavioral health care, and specialty care. Document in electronic medical records per policy and standards.
- Share the responsibility with other Street Medicine Team staff to drive and park a Sprinter van to various sites as well as help with loading outreach and medical supplies on and off van and to help to fill and empty water tanks. (Training is provided for driving the van). Provide occasional coverage for other Mobile Medical teams.
- Conduct STI assessment and testing, provide vaccinations (including COVID vaccines), wound care, point of care testing, and provision of test results.
- Provide birth control and pregnancy options counseling (including referrals for pregnancy termination services).
- Assess and counsel clients regarding opiate use disorder and medication assisted treatment. Train clients on the use of naloxone and dispense naloxone.
- Provide expertise regarding public health nursing scope, practice and policy at many levels.
- Participate in professional development opportunities and attend staff/team meetings.
- Provide program planning, monitoring local work plans and progress toward program goals as well as p articipate in quality assurance activities to meet laws, regulations, policies and procedures.
- Provide training, orientation and education to staff, clients, students and the community.
- Assist with tracking client engagement and connections to community health agencies. Adhere to safety policies and procedures.
- Other duties as assigned.
Experience, Qualifications, Knowledge, Skills
Requirements:
- Bachelor's degree in nursing science or a graduate level nursing degree of MSN, DNP or PhD from an accredited school of nursing.
- Current Washington State license as a Registered Nurse or Multi-State Licensure.
- Documentation of current BLS-CPR for Health Care Providers. CPR/AED HeartSaver courses do not meet this requirement.
- Washington state driver's license.
- Pass background check, credentialing and pre-employment physical.
