Public Health Nurse - Nurse Family Partnerships

SUMMARY:
The Department of Public Health has an opening for a Public Health Nurse (PHN) to join our Kent Nurse-Family Partnership (NFP) Team. Nurse-Family Partnership is a community health program that helps transform the lives of low-income mothers pregnant with their first child. The program partners' mothers with registered nurses from pregnancy through a child's second birthday, allowing nurses to deliver the support first-time moms need to have a healthy pregnancy, become knowledgeable and responsible parents, and provide their babies with the best possible start in life. The relationship between mother and nurse provides the foundation for strong families, and lives are forever changed--for the better. Components of the program include a caseload of 25 families per PHN, home visits with families from pregnancy until their child turns 2 years old, and weekly reflective supervision sessions and active participation in weekly team meetings
If not currently trained, the selected candidate for this project will participate in a week long training on the Nurse-Family Partnership Program in Denver, CO. in April 2020.
Responsibilities include independent fieldwork, outreach, caseload management, assessment, care planning, documentation, care coordination, and patient education, using culturally respectful and client-centered practice. This may include services to infants with special health care needs, abused/neglected children, new refugee and immigrant families and families with complex and multifaceted social concerns.

Work Location: Kent Public Health Center located at 25742 104th Ave SE, Kent WA 98030. This is a Field Position that covers all of King County.
Work Schedule: Monday through Friday, 8:00 a.m. to 4:30 p.m. This position is subject to the provisions of the Fair Labor Standards Act (FLSA) and is overtime eligible. Please note that this position may work outside of this schedule based on operational needs.

Examples of Duties:- Obtain and document health history and make nursing diagnosis based on nursing assessment.
- Documentation requires electronic charting: EPIC, and in an NFP database.
- Provide expertise regarding public health nursing scope, practice and policy at many levels.
- Provide support services to the client that links/refers, to other Public Health programs and community providers/partners, while assuring access and continuity of care.
- Provide program planning, monitoring local work plans and progress toward program goals.
- Participate in quality assurance activities to meet laws, regulations, policies and procedures.
- Provide training, orientation and education to staff, clients, students and the community.
- Provide birth control, emergency contraception and/or pregnancy options counseling.
- Provide outreach in order to promote NFP program within the community.
Competitive Candidates will have:- Knowledge of public health nursing principles and processes; human systems; infectious disease management and control; health care system structure and function.
- Demonstrated experience working respectfully with people from diverse backgrounds

- Demonstrated ability providing reproductive health education, services, community outreach, and working with community partners and agencies.
- Demonstrated ability working in a fast-paced environment independently, multi-tasking, demonstrating attention to detail, crisis intervention and problem solving skills and effective time management.
- Demonstrated ability completing medical record documentation.
- Complex problem-solving and case management.
- Demonstrated organizational and leadership abilities.
Requirements:- Willing to provide options counseling.
- Washington State Registered Nurse License in good standing.
- Bachelors of Science in Nursing (BSN) from an accredited school of nursing or equivalent.
- Proof of Current BLS CPR Certification for Healthcare Providers (Please attach if current.)
- Washington State Driver's License
- Ability to travel within the designated territory
Desired Qualifications:- Previous experience in Nurse-Family Partnership.
- Maternal-child public health nursing experience
- NCAST certification
- Lactation trained
- Bilingual/Multicultural
- Fluency in Spanish strongly preferred.

- Public Health relies on office automation (Microsoft Office) and web-based enabled tools, therefore candidates must be proficient and comfortable with computer use to perform functions associated with on-going work.

Necessary Requirements:- The selected candidate will be required to pass a thorough background investigation. Public Health Nurses who provide direct services in Community Health Services under a DSHS service agreement will go through background checks every three years.
