Registered Nurse - Downtown Public Health Center

Our Downtown Public Health team is looking for a Registered Nurse (RN) who will work as part of a multidisciplinary team in a fast-paced Family Health Clinic offering primary care services to low-income families (newborn to seniors) with medical complexity and social vulnerability. Our multidisciplinary team includes core public health physicians, ARNPs, Swedish Family Medicine Residents and Preceptors, nurses, medical assistants, behavioral health specialist, nutrition services, interpreters, referral coordinators, administrative staff and insurance enrollment assistance staff. This RN will coordinate with multiple programs within the facility in order to provide clients with seamless care. The responsibilities include nursing triage, assessments and interventions, patient care planning, evaluation, and medical care coordination, adult and pediatric immunizations, and mental health screening. The RN also carries out prescribed treatments, administers medications, and coordinates activities with other health care staff in order to assure the orderly and safe provision of health care services. The RN facilitates and assures daily clinic activities and operations. The RN maintains the clinic environment, coordinates the patient care team efforts and directs efficient clinic flow. The RN delegates tasks to the health care team within the scope of practice to achieve desirable patient outcomes and efficient clinic functioning.

Work Location and Schedule: This is a fully benefited, Career Service position at our Downtown Public Health Center which is located at 2124 Fourth Avenue Seattle, WA 98121. This typical schedule is Monday to Friday 8:30 AM to 5:30 PM. Job Duties:- Conducts nursing assessments and interventions, patient care planning, evaluation and medical care coordination.
- Maintains the clinic environment, coordinates the patient care team efforts, delegates task and directs clinic flow.
- Carries out prescribed treatments, administers medications and vaccinations, monitors and documents treatment progress and patient response.
- Conducts medical screening and triage with patients over the phone and in person.
- Obtains and documents health history, chief complaint, vital signs, growth monitoring data, assessment data, and developmental screening data.
- Refers to or consults with other health providers.
- Establishes relationships with individuals or families to promote interactive problem solving and provide patient education.
- Participates and assists in the training and orientation of health care professional students and new employees.
- Other duties as assigned within scope of practice.
Experience, Qualifications, Knowledge, Skills:- Knowledge of nursing process, ability to use a systematic approach to client care that includes assessment, diagnosis, planning, intervention, evaluation, consultation and referral.
- Knowledge of anatomy, physiology, pathophysiology, epidemiology, physical assessment and basic pharmacology.
- Knowledge of health care systems structure and function, (primary care services, community resources and private providers), allowing for appropriate client referrals based upon patient need and program guidelines.
- Knowledge of human systems (wellness, illness, growth and development, human behavior, psychosocial development, family and community systems) and alternative health care treatments.
- Knowledge of prescribed treatments and medical interventions, ability to monitor and document patient's treatment progress and response, refer to providers when indicated.
- Knowledge of Washington State law (WAC 18.79) relating to nursing care, federal health care regulations, Nurse Practice guidelines, Patient Bill of Rights, clinical practice guidelines for patient care, and understands community standards.
- Knowledge of chart documentation requirements, medical record forms, State and Federal laws that govern release of health care information, patient consent.
- Requires electronic charting in EPIC. Knowledge of health care technology, equipment, supplies and materials needed for medical treatment.
- Familiarity with the systems required to maintain the supplies, skill in repairing and calibrating clinic equipment.
- Knowledge of adult learning and health education techniques and principles including reproductive health education, related services information, options counseling, and emergency contraception services.
- Ability to effectively manage time, problem solve, manage multiple tasks, prioritize, intervene in a crisis and coordinate medical care.
- Ability to lead the work of others, including to delegate tasks and coordinate the patient care team effort to provide efficient clinic flow.
- Knowledge of research techniques and principals as well as quality assurance/improvement systems.
- Ability to apply OSHA/WISHA standards.
- Ability to communicate effectively orally and in writing.
- Ability to use information management systems.
- Ability to initiate appropriate crisis intervention and emergency response.
- Ability to maintain working relationships and ability to work with diverse populations and cultures; ability to engage clients in a care plan.
- Ability to conduct patient triage, both in person and on the phone.
- Ability to work collaboratively with multidisciplinary teams to improve patient outcomes.
- Active and unencumbered Washington State Registered Nurse license
- Current BLS for Healthcare Provider Certification at time of hire
- Minimum of Associate Degree in Nursing from an accredited School of Nursing
- Bilingual and/or Bicultural

- Public Health relies on office automation (Microsoft Office) and web-based enabled tools; therefore, candidates must be proficient and comfortable with computer use to perform functions associated with on-going work.

- Staff may be required to play an active role in the event of a public health emergency, which may include changes in responsibilities and working hours.
