Our Public Health Primary Care at Navos team is looking for an experienced, skilled, and trauma-informed Registered Nurse (RN) who will work as part of our multidisciplinary primary care team. This patient-centered medical home/primary care practice is embedded in Navos Mental Health and Wellness to provide acute care, chronic disease management, preventive care, family planning, sexual health services, and low-barrier medication treatment for opioid use disorder (MOUD) to low-income and marginalized adults in Burien, Washington. As a medical home for many individuals living with serious mental illness, substance use disorders, and homelessness, Public Health Primary Care at Navos collaborates closely with Navos Mental Health and Wellness to provide compassionate and comprehensive primary care.
This RN will partner with patients and their care teams from multiple programs within and beyond the facility to provide proactive care that is patient-centered and trauma-informed. The responsibilities include nursing triage, patient assessments and interventions, patient care planning, medical care coordination, administration of medications and immunizations, population health management, and patient and community education. Patient care occurs in person, via telephone, and via telemedicine. The RN facilitates safe, high-quality patient care by maintaining the clinic environment, coordinating patient care team efforts, directing efficient clinic flow, and delegating tasks to the health care team within the scope of practice.

- Conducts nursing assessments and interventions, patient care planning, evaluation, and medical care coordination.
- Maintains the clinic environment, coordinates the patient care team efforts, delegates tasks and directs clinic flow.
- Carries out prescribed treatments, administers medications and vaccinations, monitors and documents treatment progress and patient response.
- Conducts medical, dental, and psychiatric screening and triage with patients using a variety of modalities; in person, over the phone, or video visits.
- Obtains and documents health history, chief complaint, vital signs, patient-reported health data, and physical assessment.
- Refers to or consults with other health providers.
- Follows established Public Health medical records policies and procedures and the State and Federal laws that govern the release of health care information.
- Establishes relationships with individuals or families to promote interactive problem solving and provide patient education.
- Delegates as allowed under the Nurse Practice Act.
- Participates and assists in the training and orientation of health care professional students and new employees.
- Uses health care data for acute and chronic illness management, improvement of population health measures, and continuous practice improvement.
- Leads and facilitates panel management meetings for the work group based on health outcome data.
- Provides for clinic maintenance of supplies and equipment; maintains a safe environment according to standards of (OSHA/WISHA).
- Provides patient care based upon practice guidelines, community standards of care, and Federal and State laws and regulations.
- Maintains professional competence by pursuing opportunities for continuous learning, attending continuing education offerings, reading professional literature, and engaging in other activities which develop new knowledge and skills.

Experience, Skills, Abilities:
- Knowledge of nursing process, ability to use a systematic approach to client care that includes assessment, diagnosis, planning, intervention, evaluation, consultation, and referral.
- Knowledge of anatomy, physiology, pathophysiology, epidemiology, physical assessment, and basic pharmacology.
- Knowledge of health care systems structure and function, (primary care services, community resources and private providers), allowing for appropriate client referrals based upon patient need and program guidelines.
- Knowledge of human systems (wellness, illness, growth and development, human behavior, psychosocial development, family, and community systems) and alternative health care treatments.
- Knowledge of prescribed treatments and medical interventions, ability to monitor and document patient's treatment progress and response, refer to providers when indicated.
- Knowledge of Washington State law (WAC 18.79) relating to nursing care, federal health care regulations, Nurse Practice guidelines, Patient Bill of Rights, clinical practice guidelines for patient care, and understands community standards.
- Knowledge of chart documentation requirements, medical record forms, State and Federal laws that govern release of health care information, patient consent.
- Knowledge of health care technology, equipment, supplies, and materials needed for medical treatment.
- Knowledge of adult learning and health education techniques and principles including reproductive health education, related services information, options counseling, and emergency contraception services.
- Ability to effectively manage time, problem solve, manage multiple tasks, prioritize, intervene in a crisis, and coordinate medical care.
- Ability to lead the work of others, including delegation and coordination of the patient care team effort to provide efficient clinic flow.
- Knowledge of research techniques and principals as well as quality assurance/improvement systems.
- Ability to apply OSHA/WISHA standards.
- Ability to communicate effectively orally and in writing.
- Ability to use information management systems.
- Ability to initiate appropriate crisis intervention and emergency response.
- Ability to maintain working relationships and work with diverse populations and cultures; and engage clients in a care plan.
- Ability to conduct patient triage, both in person and on the phone.
- Ability to work collaboratively with multidisciplinary teams to improve patient outcomes.
- Washington State Registered Nurse License in good standing.
- Bachelor of Science in Nursing (BSN) from an accredited school of nursing or equivalent.
- Proof of Current BLS CPR Certification for Healthcare Providers (Please attach if current.)
- Washington State Driver's License or ability to travel within the designated territory.
- Experience providing trauma-informed care to vulnerable patients with histories of homelessness, behavioral health conditions, incarceration, and substance use.
- Experience providing patient-centered primary care including population health management.
- Ability to work creatively and flexibly with patients and colleagues to build trust and achieve goals.
- Possesses an optimistic positive attitude and approach when dealing with ambiguity and change management.
- Demonstrated experience working in multidisciplinary health care teams in a trauma-informed setting.
- Demonstrated commitment to equity and social justice and ability to foster a racially and culturally inclusive workplace.
- Demonstrated ability to understand and navigate complex systems in health and social services, including creation of community partnerships to improve patients’ access to care.
- Effective oral and written communication skills.

- Experience providing medication treatment for opioid use disorder in low barrier outpatient setting.
- Experience providing gender-affirming care and PrEP.
- Charge nurse experience or equivalent
- Minimum 2 years Ambulatory Care nursing

Work Schedule: This is a permanent, Career Service 1.0 FTE position that will work Monday through Friday 40 hours a week 8:30-5pm with the potential of working a late day (10:30am-7pm) one day a week. This position is overtime eligible and may work outside of the typical schedule to meet operational needs.
Work Location: The primary worksite for this position is at Public Health Primary Care at Navos; 1210 SW 136th St., Burien, WA 98166.

Telecommuting
If any work associated with this position may be performed through telecommute, candidates will be notified in the interview process.
