Public Health provides healthcare to the incarcerated population located in the King County jails King County Correctional Facility (KCCF) in Seattle, WA, and Regional Justice Center (RJC) in Kent, WA. Services include medical, dental, nursing, pharmaceutical, mental health and treatment of substance use disorders. With a focus on transition from jail, social services provide discharge planning for individuals with complex needs who would benefit from referral and linkage to community services.
This position will manage and oversee re-entry resources and program materials provided to individuals who are receiving JHS social services. This position will develop, implement, and maintain workflows, policy and procedures and systems to ensure re-entry kits, public entitlements, program enrollment and resources are available for individuals returning to their community.

This position works inside of the Maleng Regional Justice Center (MRJC) in Kent and the King County Correctional Facility (KCCF) *Primary Location* in Seattle and requires travel between facilities. This position may be needed to telecommute as business needs determine.

Job Duties
- Coordinate and facilitate meetings for internal and external partners.
- Coordinate and assist with developing communication materials and performing outreach to internal and external partners.
- Coordinate activities assigned in collaboration with internal programs and workgroups.
- Develop, communicate, organize, and maintain policies and procedures, workflows, and job aids relevant to the team's work and to program area.
- Schedule and organize training sessions with staff.
- Maintain the program's SharePoint page, multiple Microsoft Teams sites, and shared email accounts.
- Provide technical assistance.
- Collect, monitor, analyze and report on patient enrollment, participation, and linkage to internal and external resources.
- Create and maintain data collection procedures, daily management boards, and program performance indicators.
- Build and maintain appropriate supply of patient materials, program resources and supplies for the team.
- Serve as program custodian for gift cards and other cash-like incentives and perform quarterly reconciliations according to county procedures.
- Respond to inquiries related to program or project.
- Develop program recommendations.
- Demonstrated proficiency using cloud-based applications such as Microsoft 365 including SharePoint, OneNote, and Teams.
- Demonstrated proficiency using Microsoft Outlook, Word, Excel, PowerPoint, and Publisher to generate professional-looking program materials, reports and presentations.
- Demonstrated experience in gathering data and reporting on program services.
- Demonstrated experience developing, maintaining, distributing, and updating program materials and tracking systems independently.
- Perform analysis to support budget and program recommendations.
- Assign work to program/project assistant.
- Demonstrated skill in working and building rapport with people from diverse cultural, socio- economic and ethnic backgrounds.
- Skill in working effectively as a team member to coordinate accomplishment of specific tasks
- Demonstrated emotional intelligence to work in team environment and use conflict management skills appropriately.
- Ability to manage multiple tasks, while remaining organized and detail oriented.
- Demonstrated ability to work effectively with patients, correction staff, health staff, community, and county partners.

- Ability to travel on a flexible schedule as needed.
- Must possess valid Washington Driver's License to drive a county vehicle.
