Summary:
Under the direction and supervision of the Personal Health Services Supervisor (PHSS), this position is designed to work as a member of a multidisciplinary team in providing direct care to patients seen in the TB Clinic for diagnostic services as well as treatment for latent TB infection. This position will also provide care management services to patients with latent TB infection.
Examples of Duties:- Practice under the guidance of Disease Control Officer (DCO).
- Perform history-taking, physical exams and clinical assessments.
- Maintain knowledge of CDC technical instructions on immigration health screening and perform required clinical documentation, including civil surgeon reports and completion of I-693 form.
- Order or perform appropriate diagnostic and screening tests related to Latent Tuberculosis Infection Management (LTBI). Consult with DCO when considering sputum collection to rule out possible infectious TB.
- Gather and integrate appropriate epidemiologic and clinical data (e.g. host, environment and agent factors) through team communication, data review and documentation. Conduct comprehensive assessment of patients being evaluated for LTBI (e.g. likelihood of TB infection, risk of progression to TB disease). Consult with DCO for guidance as needed.
- Follow-up on abnormal labs for LTBI patients.
- Review x-ray reports of close contacts that are completed by Harborview Medical Center (HMC) radiologists and consult with DCO on chest x-rays that are abnormal.
- Make referrals as appropriate to primary care providers; consult with DCO for a referral to specialty care providers.
- Review LTBI patient charts for prescriptions, symptom review or dismissal.
- Conduct ongoing comprehensive assessments of LTBI patients on treatment (e.g. likelihood of TB infection, risk of progression to TB disease, benefits and risks of LTBI treatment including window period prophylaxis). Consult with DCO for guidance as needed.
- Face-to-face visits with patients including monitoring progression to TB disease, and medication side effects, as well as addressing issues of adherence and motivating the LTBI patients on treatment.
- Prescribe and dispense medication, ensuring that patients understand treatment plan through verbal communication.
- Manage directly observed preventative therapy (DOPT) for patients on the 12-dose regimen, including delegation of DOPT to TBCP outreach workers (ORW), reviewing dose counts, completing necessary logs and forms, and entering necessary information into appropriate databases. Triage potential medication complications or TB symptoms as reported by the patient, MA's, RN's or ORA
- Conduct monthly evaluation visits for LTBI patients in collaboration with MA's.
- Actively collaborate with the case management team (NCM/DIS/ORW) for the purpose of TB control in the community.
- Consult with staff on clinical matters.
- Triage potential medication complications or TB symptoms as reported by MA, DIS, RN or ORW.
- Communicate with the community-based, primary care provider who evaluates and/or treats close contacts or high-priority clients (e.g. class B) identified by our program, for the purpose of assurance of continuity of care.
- Communicate care through chart documentation and verbal interactions with TB healthcare team, using established medical record forms and documentation practices, to ensure quality patient care.
- Maintain accurate and complete records of patient care and consultation.
- Complete documentation in a concise, efficient format within a safe timeframe i.e. at the time of patient visit and within the same day or early next day for late visits.
- Required TB Program documentation includes 1) LTBI assessment and treatment forms 2) TB Tracker entry and review including medication regimen and DOPT assignment entries and updates; review of index case and contact investigation profile, 3) DOT Log completion and dose count, 4) encounter form completion for patient visits and monthly DOT log billing 4) Outreach DOPT Assignment Sheet and 5) other forms as required by management.
Other Duties:- Participate in huddle to facilitate daily review/intervention of urgent situations.
- Participate in weekly case review and work with the clinic team to prepare the presentation of updates on close contacts that are on LTBI treatment, and participate in other required program meetings.
- Operate standard MS Office applications.
- Participate in quality-control, quality-improvement, and efficiency-improvement activities, continuing education, peer review, and chart review.
- Review and provide feedback on new or updated protocols and procedures within the requested timeline.
- Cover additional clinic duties as assigned when program is temporarily understaffed or undergoing critical incident.
- Performs other duties as assigned by PHSS, Program Manager and/or DCO.
- Communicate requests for planned absence information for part or full days to PHSS. This applies to all types of leave including training requests.

Work Schedule: The work days and hours may vary and will be negotiated once an incumbent is identified. This is a fully benefited, 0.6 FTE and may work outside of determined days and hours based on operational needs.

Qualifications & Requirements:- Washington State license as a Registered Nurse (in good standing).
- Washington State license as an Advanced Registered Nurse Practitioner (in good standing) with prescriptive authority.
- Completion of advanced practitioner training and board certification as a nurse practitioner within the specialty practice.
- Must have a National Provider Identification (NPI) number prior to credentialing.
- Drug Enforcement Administration (DEA) registration must be obtained prior to credentialing.
- Knowledge of advanced practice of nursing, advanced knowledge of human systems, including wellness/illness, growth and development, basic nutrition, behavioral, psycho-social and family systems; anatomy and physiology, pathophysiology; pharmacology and therapeutics and alternative treatment modalities, diagnostic testing and interpretation of results, diagnostic reasoning and clinical decision-making.
- Knowledge of health promotion, risk assessment and education techniques and principles.
- Thorough knowledge of health care system structure and function; community, professional and educational resources.
- Demonstrated ability in: mental health assessment, communication (oral, written and use of information management systems and health care technology), medical records documentation, writing orders, time management, problem solving, crisis intervention, interviewing, analytical and critical thinking, leadership and case management.
- Demonstrated ability to consult, investigate, facilitate and present effectively.
- Demonstrated ability in initiating appropriate emergency response.
- Demonstrated ability in working with diverse cultural and socio-economic populations providing therapeutic client interventions and group dynamics
Supplemental Information:- Documentation of current BLS CPR certification with AED training required at time of hire. CPR/AED Heartsaver courses do not meet this requirement.
- Public Health relies on office automation (Microsoft Office) and web-based enabled tools; therefore candidates must be proficient and comfortable with computer use to perform functions associated with on-going work.
