Registered Nurse (RN) – Float Pool Registered Nurse; This Registered Nurse will work as part of multidisciplinary teams in a fast paced Family Health Clinic offering primary care services to low-income families (newborns to seniors) with medical complexity and social vulnerability, specifically homelessness across 3 Public Health Primary Care Clinics. The multidisciplinary teams include core public health physicians, ARNPs, 2 Residency programs, a collaboration with Navos mental health clinic, nurses, medical assistants, behavioral health specialist, nutrition services, interpreters, referral coordination, a full complement of admin staff and on-site insurance enrollment assistance. Public Health has a strong nursing team with specialties in wound care, care coordination, panel management for chronic disease, triage, lactation services and immunization care. We aspire to the highest level of NCQA medical home accreditation.
Additionally, this RN position will provide leadership; facilitating and assuring daily clinic activities/operations. This RN maintains the clinic environment, coordinates the care team efforts and directs efficient clinic flow. The RN delegates tasks to the health care team within the scope of practice to achieve desirable patient outcomes and efficient clinic functioning.
Work Schedule: This position works a 0.8 FTE, 32-hour work week, Monday - Thursday. Typical schedule is 8:00 am to 5:00 pm or 9:00-6:00 pm but may vary depending on clinic assignment. This position may work outside of these hours and days based on operational needs.

Requirements:- Current Washington State Registered Nurse license
- Requires the completion of training from an accredited school of nursing, or the equivalent
- Current BLS for Healthcare Provider Certification
- Valid Washington State Driver's License
Desirable Qualifications:- Minimum 2 years Ambulatory Care nursing preferred
- BSN preferred
- Experience working with multiple age groups and working with high risk populations impacted by substance abuse, mental illness and homelessness
- Experience with EPIC
- Can demonstrate active involvement in leadership activities over the past year
Example of Nursing Duties (may include but not limited to):- Conducts nursing assessments and interventions, patient care planning, evaluation and medical care coordination.
- Carries out prescribed treatments, administers medications and vaccinations, monitors and documents treatment progress and patient response.
- Conducts medical, dental and psychiatric screening and triage with patients over the phone and in person.
- Obtains and documents health history, chief complaint, vital signs, growth monitoring data, assessment data, and developmental screening data.
- Refers to or consults with other health providers.
- Follows established Public Health medical records policies and procedures and the State and Federal laws that govern the release of health care information.
- Establishes relationships with individuals or families to promote interactive problem solving and provide patient education.
- Delegates as allowed under the Nurse Practice Act.
- Participates and assists in the training and orientation of health care professional students and new employees.
- Establishes and maintains tracking of data for acute and chronic illness management.
- Provides for clinic maintenance of supplies and equipment; maintains a safe environment according to standards of (OSHA/WISHA).
- Provides patient care based upon these practice guidelines, community standards of care, and Federal and State laws and regulations.
- Maintains professional competence by pursuing opportunities for continuous learning, attending continuing education offerings, reading professional literature, and engaging in other activities which develop new knowledge and skills.
Nursing Qualifications:- Knowledge of nursing principles and processes; human systems; infectious disease management and control; health care system structure and function.
- Knowledge of adult learning and health education techniques and principles.
- Knowledge of personal safety standards, safety techniques and principles.
- Demonstrated abilities performing comprehensive nursing assessments and successfully engaging clients in a plan of care.
- Demonstrated ability to communicate effectively with patients and their families and with other health and human services staff; developing and maintaining therapeutic relationships with patients; and working with culturally and socio-economically diverse populations.
- Demonstrated ability providing reproductive health education, services, community outreach, and working with community partners and agencies.
- Willing to provide options counseling.
- Ability to work collaboratively with multidisciplinary teams to improve patient outcomes.
- Demonstrated ability completing medical record documentation, problem-solving and case management.
- Skilled in motivational interviewing.
- Able to provide care through a lens of harm reduction, trauma informed care and equity & social justice.
- Knowledge of human systems (wellness, illness, growth and development, human behavior, psychosocial development, family and community systems) and alternative health care treatments.
- Knowledge of prescribed treatments and medical interventions, ability to monitor and document patient's treatment progress and response, refer to providers when indicated.
- Knowledge of Washington State law (WAC 18.79) relating to nursing care, federal health care regulations, Nurse Practice guidelines, Patient Bill of Rights, clinical practice guidelines for patient care, and understands community standards.
- Knowledge of chart documentation requirements, medical record forms, State and Federal laws that govern release of health care information, patient consent.
- May require electronic charting. Knowledge of health care technology, equipment, supplies and materials needed for medical treatment.
