The Posittion:
Seattle & King County Public Health is looking for a motivated Registered Nurse (RN) who is passionate about providing care to our community's most vulnerable populations through a trauma informed care (TIC) lens. Navos Primary Care opened in 2012 and continues to be an innovative model for integrated care which embeds a primary care clinic for adults, in the campus of a community mental health center. The nurse who is selected to fill this role will have an opportunity to work closely with medical providers, behavioral health therapists, case managers as well as other members of the integrated care team. The goal is to provide whole health care to persons experiencing significant and persistent mental illness who are receiving care at the Navos Mental Health and Wellness Center. Through care management, patient education and clinical support the nurse is able to help decrease profound health disparities. All members of this collaborative are committed to a patient-centered medical home model and the nurse is instrumental in the care continuum. Work Location and Schedule: Public Health Primary Care at Navos, 1210 SW 136th St., Burien, WA 98166. Typical working hours for this 1.0 FTE, Career Service position is 9:30 a.m. to 6:00 p.m. on Mondays and 8:30 a.m. to 5:00 p.m. Tuesday-Friday. Please note that travel to cover operational needs at neighboring Public Health Clinics may be required.

Examples of Duties:- The responsibilities include nursing assessments and interventions, patient care planning, evaluation and medical care coordination.
- The RN maintains the clinic environment, coordinates the patient care team efforts, delegates tasks and directs clinic flow.
- The RN also carries out prescribed treatments, administers medications and vaccinations, monitors and documents treatment progress and patient response.
- Conducts medical, dental and psychiatric screening and triage with patients over the phone and in person.
- Obtains and documents health history, chief complaint, vital signs, growth monitoring data, assessment data, and developmental screening data.
- Refers to or consults with other health providers.
- Follows established Public Health medical records policies and procedures and the State and Federal laws that govern the release of health care information.
- Establishes relationships with individuals or families to promote interactive problem solving and provide patient education.
- Delegates as allowed under the Nurse Practice Act.
- Participates and assists in the training and orientation of health care professional students and new employees.
- Establishes and maintains tracking of data for acute and chronic illness management.
- Provides for clinic maintenance of supplies and equipment; maintains a safe environment according to standards of (OSHA/WISHA).
- Provides patient care based upon these practice guidelines, community standards of care, and Federal and State laws and regulations.
- Maintains professional competence by pursuing opportunities for continuous learning, attending continuing education offerings, reading professional literature, and engaging in other activities which develop new knowledge and skills.
Qualifications:- Knowledge of nursing process, ability to use a systematic approach to client care that includes assessment, diagnosis, planning, intervention, evaluation, consultation and referral.
- Knowledge of anatomy, physiology, pathophysiology, epidemiology, physical assessment and basic pharmacology.
- Knowledge of health care systems structure and function, (primary care services, community resources and private providers), allowing for appropriate client referrals based upon patient need and program guidelines.
- Knowledge of human systems (wellness, illness, growth and development, human behavior, psychosocial development, family and community systems) and alternative health care treatments.
- Knowledge of prescribed treatments and medical interventions, ability to monitor and document patient's treatment progress and response, refer to providers when indicated.
- Knowledge of Washington State law (WAC 18.79) relating to nursing care, federal health care regulations, Nurse Practice guidelines, Patient Bill of Rights, clinical practice guidelines for patient care, and understands community standards.
- Knowledge of chart documentation requirements, medical record forms, State and Federal laws that govern release of health care information, patient consent.
- May require electronic charting. Knowledge of health care technology, equipment, supplies and materials needed for medical treatment.
- Familiarity with the systems required to maintain the supplies; skill in repairing and calibrating clinic equipment.
- Knowledge of adult learning and health education techniques and principles including reproductive health education, related services information, option counseling, and emergency contraception services.
- Ability to effectively manage time, problem solve, manage multiple tasks, prioritize, intervene in a crisis and coordinate medical care.
- Ability to lead the work of others, including to delegate tasks and coordinate the patient care team effort to provide efficient clinic flow.
- Knowledge of research techniques and principals as well as quality assurance/improvement systems.
- Ability to apply OSHA/WISHA standards.
- Ability to communicate effectively orally and in writing.
- Ability to use information management systems.
- Ability to initiate appropriate crisis intervention and emergency response.
- Ability to maintain working relationships and ability to work with diverse populations and cultures; ability to engage clients in a care plan.
- Ability to conduct patient triage, both in person and on the phone.
- Ability to work collaboratively with multidisciplinary teams to improve patient outcomes.
- Ability to provide option counseling and referral for terminations.
Most Competitive Candidiates Will Have:- 1-2 years experience working in Ambulatory Nursing Care
- 1-2 years experience working with vulnerable populations
- 1-2 years experience working in Behavioral Health
- BSN preferred.
Job Requirements:- Requires current Washington State Registered Nurse license.
- Requires the completion of training from an accredited school of nursing
- Washington State Drivers' License or the ability to provide transportation to work locations may be required.
- CPR-BLS Healthcare Providers by time of hire.

- Public Health relies on office automation (Microsoft Office) and web-based enabled tools, therefore candidates must be proficient and comfortable with computer use to perform functions associated with on-going work.
