UW Medicine Population Health Management has an outstanding opportunity for a Clinical Care Coordinator, Accountable Care Network (ACN) & Value-Based Contracts. This individual will be an experienced registered nurse focused on patient lives in contracted value based arrangements for both UW Medicine and UW Medicine ACN. The clinical care coordinator serves as the initial point of contact and gatekeeper for payer groups who refer patients to the health system. This role is dedicated to supporting our clinic based care managers and providers to achieve our targeted mission of improving the health of the public by identifying patient populations in value based contracts at risk due to lack of empanelment, utlization management, prevention/disease management, or integrated care management. May support as needed post-inpatient discharge transitional care management. The Clinical Care Coordinator can offer direct patient care coordination or provide a warm hand off to a clinic care manager for patient follow-up.
The Clinical Care Coordinator will promote improved patient outcomes by conducting centralized reviews that identify patient's correct primary care and provider attribution; refer these patients to the appropriate care manager. The individual in this role will be empowered to use competency in clinical and care management skills and experience, as well as critical thinking that is self-initiated, while simultaneously meeting contractual timelines for all payers.
The Clinical Care Coordinator will provide care coordination for patients needing access to organizations and/or services particularly in an effort to assist in avoiding unnecessary services outside of UW Medicine ACN or UW Medicine. The Clinical Care Coordinator will use internal and external health intelligence tools and payer reports to identify, to outreach, and to educate patients on how to more effectively and efficiently navigate the health system(s) to reduce utilization, to reduce cost, and to improve quality of care.
The Clinical Care Coordinator reports directly to the Director of Care Management for UW Medicine
