UW Medical Center Pharmacy is seeking a highly motivated FULL-TIME CLINICAL PHARMACIST – BONE MARROW TRANSPLANT (Rotating/Variable Shifts) to provide pharmaceutical care in our inpatient oncology patient population. The Clinical Pharmacist provides extensive pharmaceutical care, and is a vital part of the inpatient multidisciplinary patient care team. In addition to direct patient care responsibilities, our pharmacists provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transitions of care, precept pharmacy students and residents, and perform duties of a Pharmacist 2.
Primary Responsibilities:
Clinical:
Drug therapy monitoring, including, but not limited to:
• Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies.
• Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed.
• Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations.
• Provide drug information and education to medical teams, nursing, and dietitian staff.
• Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders at each visit.
Education and Research:
• Provide primary preceptorship of pharmacy residents and student pharmacists.
• Provide education to medical/surgical providers, students, and nursing staff.
• Present at educational conferences to multidisciplinary staff, as requested.
• Develop and provide nursing in-services.
• Provide didactic teaching to student pharmacists at the University of Washington School of Pharmacy, as requested.
• Participate in collaborative research projects with healthcare professionals, pharmacy residents, and student pharmacists.
• Submit unique work for publication in professional journals.
Practice Management:
• Implement and maintain departmental policies and procedures related to the safe and effective use of medications.
• Routinely evaluate current updates in service-specific literature.
• Develop and maintain drug therapy monitoring systems for patients.
• Establish and maintain efficient lines of communication between pharmacists, physicians, nurses, and other members of the healthcare team.
• Participate in pharmacy and hospital committees as requested.
• Participate in the development and implementation of quality assurance programs for pharmacy services, including, but not limited to, prevention of medication errors.
• Perform drug utilization evaluation and cost-containment analyses, as needed.
Dispensing:
• Perform duties of a Pharmacist 2.
DESIRED:
ASHP-accredited PGY2 residency in oncology pharmacy practice.
Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
