Harborview Medical Center's Mental Health and Addictions Services has an outstanding opportunity for a 100% Full-Time Mental Health Practitioner/ Housing Case Manager to assist adults with individualized housing services by developing and maintaining permanent, community based housing for clients of the outpatient mental health clinic. A Housing Specialist/Mental Health Practitioner will be expected to be able to work with a wide variety of clients and be sensitive to the culture, religion, language, age, gender and sexual orientation of those we serve.
PRIMARY JOB RESPONSIBILITIES
Assist clients of outpatient mental health services with individualized apartment searches, rental applications, subsidy paperwork, accessing community housing resources, etc.
Work with potential/existing landlords, apartment managers and community housing assistance programs to develop/maintain new existing housing relationships
Work with the Seattle Housing Authority, Shelter Plus Care Administration, Set-Aside Programs, Section 8 Program and others
Facilitate and co-facilitate housing support groups
Provide individual support services to clients
Provide consultation and training to clients and clinical staff regarding housing subsidies and various forms of housing assistance
Attend interdisciplinary team meetings as well as community meetings to discuss and promote housing services
HOUSING CASE MANAGEMENT
Maintain a caseload, size to be determined by team and client needs
Maintain Waiting List for available subsidized housing
Provide Division of Vocational Rehabilitation funded Independent Living Services
Work closely with the Employment team and Case Managers to ensure integration of services
Assist Clients with apartment searches
Assist Clients with rental applications
Provide application and advocacy services for subsidy programs (e.g. move applications, income change reports, household composition changes)
Schedule and assist with moves
Assist clients in identifying community housing resources
Work with potential landlords, apartment managers and community housing assistance programs
Work with clients’ families and social networks
Work with the Seattle Housing Authority, Shelter Plus Care Administration, Set-Aside Programs, Section 8 Program and others
Assess housing barriers and determine interventions
Monitor needs for basic living skills as it pertains to maintaining housing particularly for those on Shelter Plus Care
Coordinate the Activity of Daily Living training for those who need it to maintain independent living
Attend treatment team meetings
Consult with case managers and medical staff regarding effects of Housing activities and treatment

Maintain clinical and billing records
Submit daily billing logs to Data Systems
Chart client contact in the medical record
Develop referral information for various Housing Subsidies
Update Financial Counselor of changes in patients’ financial and housing status
Collect data for evaluation and research
TEACHING
Be responsible for informing staff and management about relevant changes in housing, trends, rules and regulations
Assist in training residents, nursing and social work students
Assist in orientation of new staff
Explain procedures to new staff, students and residents
Allow new staff and trainees to observe treatment procedures
TRAINING
Maintain State of Washington License: Registered Counselor, Certified Mental Health Counselor, and or Certified Social Worker.
Obtain training and education relevant to the needs of under-served populations particularly as it pertains to housing
Obtain continuing education to stay current with trends in mental health, regulations and other pertinent information particularly as it pertains to housing

A Master's Degree in behavioral sciences, such as psychology, psychiatric nursing, occupational therapy, vocational or educational counseling, or social work OR Bachelor's Degree in nursing or a behavioral science AND three years' of experience in the direct delivery of professional mental health care.
Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
ADDITIONAL REQUIREMENTS:
Registration as either an 1) Agency Affiliated Counselor, 2) Licensed Social Work Associate Advanced or 3) Licensed Social Work Associate Independent Clinical by the State of Washington within 90 days of employment
DESIRED
Demonstrate working knowledge of local housing resources including, but not limited to, Shelter Plus Care, Section 8, HUD, SHA and local Housing Organizations.
Willingness/ability to develop new housing partnerships within Seattle and King County
Knowledgeable about tenant/landlord law/ knowledge of local, state and federal eligibility guidelines for housing.
