The HMC Pain Relief Clinic has an outstanding opportunity for a Patient Care Coordinator (Full-Time, Day Shifts). The Patient Care Coordinator (PCC) is knowledgeable of hospital policies and procedures, insurance coverage requirements, including authorization and third-party payer and reimbursement procedures. The PCC coordinates all patient services such as admitting, transfer to/from other hospitals, discharge appointments, arranging diagnostic procedures, scheduling surgery and consultation requests, and scheduling services from other hospital departments in order to facilitate delivery of care to patients. The PCC works closely with the RN3, physicians, and medical staff to ensure a high standard of care and strives to continually improve the patient experience and deliver excellent customer service.
Primary responsibilities:
Procedure Scheduling and Care Coordination:
•Serve as the primary liaison for patients and physicians regarding surgical coordination, ordering diagnostic studies, admission process, and disability coordination
•Coordinate inpatient and outpatient care by scheduling surgery, arranging diagnostic procedures and/or coordination requests for consultation among other medical specialties
•Maintain surgery schedules for assigned surgeons and coordinates schedule with Anesthesia, Admitting, and the Operating Room
•Coordinate communication among patient, orthopedic surgeons, and internal and external providers
•Facilitate patient care through the initiation, coordination, and completion of health plan authorizations and pre-op and admission packets
•Perform telephone assessment of patient problems within scope of practice and refer patients to RNs and physicians as appropriate
•Document patient encounters regarding financial or clinical issues on appropriate forms
•Coordinate and ensure needed clinical and administrative information is available for physicians when needed
Patient demographic and financial information:
•Collect demographic data when scheduling patients for surgeries and tests and refer to financial counselor for updating
•Obtain necessary authorizations from PCPs and health plans for surgeries and tests
•Educate patients about their responsibilities for preparing for surgery including pre-anesthesia instructions, advance directives, and health plan approval
•Refer patients to health coverage services as necessary to address funding issues
Communication and Teamwork:
•Maintain clear lines of communications for efficient and effective teamwork with all staff and physicians
•Be a leader in conflict resolution and provide constructive feedback to members of the team
•Communicate regularly with the Clinic Practice Manager about work issues, process improvement ideas, and problem-solving
•Coordinate communication among PCCs, Ambulatory Service Representatives, and nursing staff
•Maintain a positive working relationship with co-workers and physicians
•Orient physicians, residents, medical students, and new staff to surgery scheduling and the admission process in clinic
•Perform related duties as required.

•Demonstrate ability to communicate information in an effective manner, including as needed to educate patients
•Demonstrate competency in providing care for the typical age ranges of patients seen in clinic
•Willingness and ability to develop and maintain successful interpersonal relationships with all staff and physicians
