The Harborview Otolaryngology Clinic has an outstanding opportunity for a Full-Time Patient Care Coordinator (100%, Day Shifts). The Patient Care Coordinator (PCC) organizes outpatient care by scheduling surgeries, admissions, diagnostic procedures, interpreters, and/or coordinating requests for consultation among other medical specialties by utilizing specific directions from providers. The PCC acts as the primary contact for clinic providers with the hospital in/out patient units, patients and families, referring physicians, outside agencies, staff from outside hospitals, and general public by providing information and guidance regarding Harborview Otolaryngology and Medical Center services
Primary responsibilities:
•Act as primary liaison for the patient with hospital, referring provider, insurance carrier, and outside agencies and staff from other health providers to coordinate activities involved in pre-operative planning
•Assist patients with completing insurance forms and requests for information by providing documentation to facilitate coverage for scheduled visit, admission and/or procedure
•Manage referrals in a complete and timely manner with special attention to urgent conditions/requests. Must be able to prioritize multiple competing projects and be able to meet deadlines
•Pre-authorize proposed healthcare/surgery prior to admission
•Coordinate supporting documentation to facilitate and coordinate patient care involving agencies outside of HMC
•Coordinate pre-admission tests, pre-surgery appointments/tests, and procedures
•Complete appropriate documentation for planned admissions and surgeries
•Manage records confidentially in accordance with medical center policies/procedures and RCW’s
•Schedule complex surgeries and/or procedures, which includes day surgeries and those requiring hospital admission. This may involve coordination with other specialty services such as Neurosurgery, Burns/Plastics, Vascular, and others
•Prepare pre-anesthesia packets and assures related electronic forms are completed
•Coordinate pre-admission tests, pre-surgery appointments/tests, and procedures
•Arrange for special equipment that may be required for specific surgeries
•Maintain an organized method for tracking patient care along continuum
•Assist patients in coordinating services and scheduling pre and post-operative appointments
•Ensure a complete medical record is present at the time of patient visit, admission, and/or procedure
•Manage intake of all referral requests, ensuring complete information is received and entered into the computer system
•Triage patient phone calls according to severity of need; follow through and resolve patient requests in a timely manner
• Build/manage Provider and Resident templates via Epic application
