HARBORVIEW MEDICAL CENTER PHARMACY has an outstanding opportunity for a TEMPORARY/HOURLY PHARMACIST 2 (Rotating/Variable Shifts). The Pharmacist 2 will perform clinical and distributive pharmacy duties in any one of the five outpatient pharmacies, and will provide drug information and education to patients, physicians/ clinicians, and nurses. The Pharmacist 2 instructs students, residents, interns, and technicians, and provides pharmaceutical care to patients.
Responsibilities:
Distribution:
• Prepare and dispense medications.
• Conduct a thorough patient profile review to assess the appropriateness of the prescribed pharmaceutical, including monitoring individual patient profiles for drug duplications, interactions, allergies, appropriate dose, strength, and route of administration at the time each order is received. Document relevant information in patient medical record.
• Evaluate all physicians' orders and verify correctness of entries in computerized patient profiles.
• Interview patients and provide education regarding their medications.
• Comply with all pharmacy laws and regulations.
• Provide continuity for daily operations and patient interactions.
• Direct the work of technicians, interns, pharmacy students, and residents.
• Maintain accurate records of controlled substances dispensed.
Clinical:
• Perform direct patient screening and education on selected medical conditions in consultation/counseling rooms in the outpatient pharmacy waiting room.
• Recommend appropriate drug therapy based on patient parameters and interpretation of current literature, safety, efficacy, and cost.
• Advise providers about patients' drug therapy.
• Perform refill authorization for selected ambulatory clinics. Review patient history, assess current drug therapy for appropriateness, compliance, duration, and duplication of therapy. Provide documentation in medical record and communication to prescriber as outlined in prescription refill protocol.
• Maintain accurate medication histories and monitor effect and adverse reactions.
• Participate in the implementation of formulary management, optimizing patient care through appropriate selection of treatment options.
Educational:
• Assist with the instruction of students, residents, and staff pharmacists regarding clinical pharmacy, distribution, and drug information.
• Participate in regularly scheduled pharmacy continuing education provided by the department.
• Provide drug information and/or periodic in-services to physicians, nurses, patients, and pharmacy staff.
Other Duties:
• Assist with periodic peer review and evaluation.
LEGAL REQUIREMENT: State of Washington Pharmacist license.
