UW Medical Center Pharmacy has an outstanding opportunity for a FULL-TIME PHARMACIST 3 (Day Shifts) to work within the Anticoagulation Clinic. The clinical pharmacists in the UWMC Anticoagulation Clinic manage anticoagulant therapy for ambulatory patients, working with a team of specialists in antithrombotic therapy and multidisciplinary providers from numerous specialties. The UWMC Anticoagulation Clinic is a pharmacist-based patient management clinic that provides consultation services to our hospital medical staff and also serves UW patients within the region.
Responsibilities:
Clinical:
Drug therapy monitoring, including, but not limited to:
- Implement, monitor, and adjust antithrombotic pharmacotherapy for achievement of desired therapeutic outcome.
- Ensure appropriate drug therapy, including indication, dose, side effects, and drug allergies.
- Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events; intervene as needed.
- Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations.
- Provide patient education or initiation of anticoagulant therapy, including bridge therapy and management of drug interactions and adverse outcomes.
- Provide anticoagulation dosing recommendations, drug information, and education to medical teams, nursing, and other healthcare students and staff.
- Provide patient/family counseling and education.
Education and Research:
- Provide primary preceptorship of pharmacy residents and student pharmacists in the Anticoagulation Clinic.
- Provide education to medical/surgical providers and students, and nursing staff.
- Present at educational conferences to multidisciplinary staff, as requested.
- Develop and provide nursing in-services as requested.
- Provide didactic teaching to student pharmacists at the UW School of Pharmacy, as requested.
- Participate in collaborative research projects with healthcare professionals, pharmacy residents, and student pharmacists.
Practice Management:
- Implement and maintain departmental policies and procedures related to the safe and effective use of medications, including anticoagulation therapies.
- Participate in managing transitions of care among various healthcare settings and protocol development.
- Routinely evaluate current updates in service-specific literature.
- Develop and maintain drug therapy monitoring systems for patients.
- Establish and maintain efficient lines of communication among pharmacists, providers, nurses, and other members of the healthcare team.
- Participate in pharmacy and hospital committees as requested.
- Participate in the development and implementation of quality assurance programs for pharmacy services, including, but not limited to, prevention of medication errors.
- Perform drug utilization evaluation and cost-containment analyses, as needed.
Dispensing:
- Perform the duties of a Pharmacist 2.
Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
DESIRED:
- Previous anticoagulation experience.
- Completion of an accredited residency training program (PGY1/PGY2).
