The HMC Rehabilitation Medicine Clinic has an outstanding opportunity available for a full time day shift PROGRAM COORDINATOR. Under Business Operations Manager Supervision, the Rehabilitation Medicine Program Coordinator (PC) works within the AACS clinical practice area to facilitate necessary care for complex Rehabilitation Medicine patients including but not limited to:
Patients with work related injuries resulting in limb loss that will be managed in the outpatient Amputee L&I Center of Excellence at Harborview; patients with spinal cord injuries (SCI) and pressure ulcers requiring multidisciplinary care including surgical evaluation and intervention (FLAP Program).
Particular goals are to coordinate the delivery of and access to the spectrum of medical, nursing, and clinical care services - as formulated by the medical staff –for patients with work related limb loss injuries referred to the Rehabilitation Amputee L&I Center of Excellence (COE) and for patients with SCI injuries referred to SCI Rehabilitation Medicine clinic and the FLAP program. Part of the program coordinator role will be to function as an HSC (Health Services Coordinator) to improve clinical outcomes of injured workers by expediting necessary correspondence and communication among workers’ compensation stakeholders including: injured worker, care provider, claim manager, and employer, to prevent administrative delays that contribute to short and long-term disability.
Position requires a high degree of autonomy, personal initiative, discretion, forethought, flexibility, strong communication skills, and independent judgment needed to organize and prioritize tasks to ensure deliverables and deadlines are met. Attention to detail is critical, as is the ability to think critically and independently problem-solve challenges. A strong commitment to excellence in work product and an active interest in providing customer service is expected.
MAJOR RESPONSIBILITIES:
Identify, Track and Coordinate Services for L&I Workers Compensation Claims
• Screen workers’ compensation claims and identify claims that will benefit from health services coordination.
• Help providers, injured workers, and employers navigate the workers’ compensation system.
• Facilitate access to workers’ compensation claim benefits and programs such as Stay at Work, Early Return to Work, vocational counseling, and other processes through communication with employer, patient, and other stakeholders.
• HSCs must write accurate case notes which document and bill for services provided. HSC case notes must include specific fields and a designated formatting within our electronic medical record and billing system.
• Access the L&I claim file through L&I’s Claim and Account Center (CAC). Utilize and post EHR case notes into L&I’s Occupational Health Management System.
• Facilitate the flow of documents between patients, providers, employers and claim managers and billing.
• Contact L&I staff as needed for follow-up or assistance on specific claims.
o Staff includes: claim managers, occupational nurse consultants, vocational or therapy staff in the field offices, risk management, DOSH staff, and/or any others that can provide needed support to providers, workers, or employers.
Communication, Care Coordination and HSC Services
• Act as a liaison, on behalf of attending providers in the Rehabilitation Amputee L&I COE between the injured worker, employer, and claim manager.
• Coordinate care needs for patient, including access to primary and specialty care and provide support to the patient/family by locating appropriate staff to address concerns.
• Maintain contact list of key clinical partners, including roster of participating providers
• Interface with Health Information Management and Release of Information to ensure records are accessed and released in accordance with HMC privacy policies.
• Develop optimal procedures for tracking and proactively addressing communication and care coordination needs for injured workers.
• Track specific claims in order to ensure early return-to-work services, care coordination, and improved clinical outcomes of injured workers, as appropriate.
• Contact L&I staff as needed for follow-up or assistance on specific claims.
o Staff includes L&I’s claim managers, occupational nurse consultants, vocational or therapy staff in the field offices, risk management, DOSH staff, and/or any others that can provide needed support to providers, workers, or employers.
• Facilitate the flow of documents between patients, providers, employers and claim managers and billing.
• Manage high volume of correspondence pertaining to preauthorization and ongoing authorization processes, patient scheduling, and patient/provider follow up requests.
• Work independently to determine priority for patient requests and clinical follow-up needs. Follow through and resolve patient requests on a timely basis.
• Coordinates patient care for a comprehensive, intensive outpatient rehabilitation program which is unique to the local and broader community. Patient population is very complex both medically and socially, with neurological diagnoses of multi-trauma including limb loss and spinal cord injury.
• Complex referral management, authorization management, and scheduling of appointments for services such as social work, nutrition, rehab psychology, occupational therapy/ physical therapy, prosthetics and orthotics, complex wheelchair seating evaluations and other durable medical equipment needs.
• Coordinates pre-surgery care for the flap protocol patients by obtaining the appropriate authorizations for disciplines of a multi-disciplinary team.
• Coordinates discharge from acute care units including 4WH (Rehab) with discharge team and assists in the inpatient to outpatient transition. Communicates with discharge planner (4WH) or nurse coordinator (CCN) on the acute care units regarding referrals and benefit information for a smooth transition.
• Tracks referrals to other services to ensure that referral recommendations are followed and is responsible for gathering medical paperwork, such as disability and compensation paperwork.
• Coordinates patient and team rounds and conferences for physicians and comprehensive clinic team.
• Direct transfer of patient related information to and from health providers including in-house and outside referring entities.
• Maintain continuity of care by assessing follow up needs, e.g., appointments, transportation, transfer of records, locating providers, facilitating filing of claims etc.
• Maintain knowledge of pertinent medical diagnostic procedures and related procedural instructions and guidelines and medical record documentation requirements, documenting in medical record when appropriate. E.g., Schedules complex procedures that require coordination with Radiology, such as CT, MRI, Ultrasound. Responsible for pre-authorization for study and scheduling with patient and Radiology.

Minimum Qualifications:
• High school graduation or equivalent, AND
• Two years of experience in the program specialty, OR equivalent education/experience.
Desired Skills:
• A bachelor’s Degree in a medically related or public relations field and two years of experience in a tertiary patient care setting involving interaction with a variety of medical services, contact with patients and families, interaction with physicians and proficiency in medical terminology
OR equivalent education/experience.
