Under the general supervision of the department director, reporting to the Social Work Supervisor, this position serves as a member of an integrated, multidisciplinary team that is focused on the assessment, support and/or post-acute disposition of NWH patients and their families. This position will seek opportunities to create improvements in resource utilization, length of stay and appropriate discharge planning criteria. These duties will be performed in accordance with, and in adherence to, the mission, vision, and values of Northwest Hospital.
Sr Social Worker MSW (NE H SEIU 1199NW UWMC Northwest Professional)
Under the general supervision of the department director, reporting to the Social Work Supervisor, this position serves as a member of an integrated, multidisciplinary team that is focused on the assessment, support and/or post-acute disposition of NWH patients and their families. This position will seek opportunities to create improvements in resource utilization, length of stay and appropriate discharge planning criteria. These duties will be performed in accordance with, and in adherence to, the mission, vision, and values of Northwest Hospital.
Completes accurate and thorough assessment of patient and family psychosocial functioning with focus on potential barriers to discharge.
Provides emotional support, crisis counseling as needed; mental health and substance abuse interventions.
Communication/Collaboration, includes but is not limited to:
Collaborates with case manager to provide for patient needs according to individual patient/condition requirements and care setting.
Establishes and maintains collaborative relationships with the health care team. Ensures timely coordination with other care management staff to ensure continuity as patients move throughout the hospital.
Communicates effectively in writing, over the telephone, and in person with medical staff, nursing staff, ancillary support staff, external referral sites, patients, and families.
Accurately and appropriately documents all assessments, interventions and discharge plans.
Supports a work climate that is conducive to recruitment and retention of staff. Maintains objectivity and good interpersonal skills that promote effective interaction with a wide variety of people. Remains calm and constructive in conflict situations.
Participates in department and committee meetings, unit rounds, and patient/team conferences as appropriate.
Discharge Planning, includes but is not limited to:
Initiates discharge planning from admission and through discharge;
coordinates required resources for patient/family to achieve safe and timely discharge/transfer.
Demonstrates knowledge and appropriate use of hospital and community resources.
Prioritizes with the case manager to facilitate timely discharge planning.
Facilitates patient placement to appropriate disposition; arranges needed equipment and/or transportation, and ensures arrangements for appropriate follow up.
Payor and Regulatory Criteria, includes but is not limited to:
Demonstrates a working knowledge of Utilization Management
Maintains a working knowledge of all local, state and federal guidelines, requirements, and regulations
Knowledgeable of length of stay associated with specific diagnoses and implements timely discharge planning appropriate for length of stay.
Process Improvement, includes but is not limited to:
Identifies operational problems and takes action to correct specific problems.
Identifies patients who are readmitted and evaluates care and discharge needs if necessary.
Participates in process improvement efforts.
Certifications:L-LW | SOCIAL WORKER ADV LIC - WA State (required)
Requires Washington State Registration
Masters degree in Social Work (MSW) required
DESIRED
Prefer Independent Social Worker (LSW) and Washington State License.
One to two years acute medical experience with discharge planning and resource utilization strongly preferred
Familiarity with Geropsych
