Heart Failure –Transplant Inpatient Service
Faculty Appointment: The position of Teaching Associate/Advanced Practice Provider (APP) without tenure in the Division of Cardiology, Department of Medicine, is a one-year appointment, renewable annually (JCC: 0169).
Job Description: The Heart Failure –Transplant Service APP is a member of an interdisciplinary care team providing inpatient and outpatient services to patients with advanced heart failure and following heart transplantation in a non-critical care setting (Cardiology B) at UWMC. The APP will be responsible for providing primary direct patient and work collaboratively with other team members, including a team of APPs, attending cardiologists, fellows and residents. The position requires a background and education necessary to obtain appropriate clinical privileges, and skills that include the provision of initial and ongoing assessment, diagnosis and treatment of the patient’s medical, physical and psychosocial status.
Reports to: Medical Director and Associate Medical Director of the Advanced Heart Failure Subsection, and the Chief of Cardiology
MINUMUM QUALIFICATIONS:
1. Current and valid licensure as an ARNP or PA-C in Washington State
2. Graduate Degree from an accredited Nurse Practitioner Program or graduate of an accredited Physician Assistant program, or foreign equivalent.
3. Nationally certified ARNP/PA with DEA
4. Proven excellence working in an interdisciplinary collaborative team environment
5. Excellent interpersonal skills, leadership skills, and clinical expertise in the care of patients with chronic and acute cardiovascular illnesses.
STANDARDS OF PRACTICE AND PERFORMANCE
Patient Care
The APPs are expected to be the primary leader in the care of patients on the Card B service. As a result, they are prepared and act as the go-to person for the patients under their care. In order to provide the highest level of comprehensive care to this complex patient population, the APPs will:
a. Pre-round on patients prior to interdisciplinary rounds. Pre-rounding involves physically seeing patients, especially those patients who may need urgent attention in order to execute the plan of the day with the most accurate data.
b. Prioritize based upon pre-round assessments the order of rounds, based on patient acuity.
c. Round collectively with all members of the team to generate an efficient multidisciplinary rounds.
d. Perform brief afternoon rounds on all patients prior to the 4pm brief
e. Attend the 4pm brief with the attending to discuss next day planning, new admits, strategize patient flow, discuss additional resources, revisit and finalize plan of the day
f. Support fellow APPs during time of unexpected emergencies where coverage of the service will be affected.
g. Communicate patient safety issues with attending. This includes soliciting assistance due to heavy workload
h. Request and follow-up on consultations with specialty services
i. Initiate transitions of care and coordinate transfer to other services as part of the discharge planning process
j. Lead, monitor, and reinforce practice guidelines and promote the use of clinical care maps and standardized protocols.
k. Educate staff, fellows, other APPs, patients, and families
l. Participate in care conferences and family meetings
m. Acquire, maintain, and apply current knowledge of the practice of advance heart failure therapies.
Communication
a) Communicate pertinent clinical information in a timely manner
b) Facilitate collaboration
c) Promote and enhance communication with patients, family members, and members of the interdisciplinary team
d) Provide accurate and timely verbal hand-offs.
e) Serve as a point person for communications with consulting services for assigned patient panel
f) Communicate in a professional and respectful manner, at all times, with all members of the health care team whether through verbal, email, or text communication
g) Provide accurate and timely order entry via ORCA CPOE
h) Practice and promote the practice of communication with TeamSTEPPS elements with all interprofessional team members
Professional
i) Participate in research and lead quality initiatives
j) Provide education and mentorship to 5SE/5SA/5NE staff, fellows, residents, nurses, patients, and families
k) Participate in UW-based professional committees
l) Thoughtfully and accurately present relevant clinical data in a problem-based format when rounding or in identifying critical changes that need appropriate and timely follow up
m) Report critical changes in a timely manner
n) Follow UWMC Professional Behavior Policy and Patients are First Guidelines
SCOPE AND HOURS OF WORK
Following a training period of 30 – 90 days (varying depending on background, previous training and experience), the candidate will participates in daily rounds, admission, daily care and discharge of inpatient heart failure and transplant patients. The APP will work a full-time schedule that includes approximately twelve 12 hourshifts per month. The Cardiology B service is covered by APPs seven days a week from 0800-2000. The chosen candidate will be expected to attend weekly Cardiology Grand Rounds and various monthly research and clinical case conferences.
